Need A Sick Note?

How long have you been ill?

7 Days or Less

As you have been ill for 7 days or less you don't need to see a Doctor. You can complete a Self Certification form yourself. Please click on link to open a PDF version which you can print or clink on the link to go directly to the GOV.UK website.

However some employers insist on a Doctor's note regardless. In this instance you will require what is called a Fit Note. Your GP is entitled to charge you for a Fit Note when the illness has been for 7 days or less. Our fee for this service £15. If this is the case and you have visited the surgery then all you would require is a telephone consultation. If you have not visited the surgery then you would require either a face-to-face or telephone consultation.

Is this your first Sick Note for this period of illness?

If this is not the case and you have already had a sick note for this illness, you may not need to see your Doctor to receive a subsequent Sick Note. You can have a telephone consultation. 

If it is your first sick note, then you would need to book a face-to-face consultation using our online services or via reception.
